should be in the name of:

• Your purchase order reference number & signed by the officer
authorizing to purchase.
• Your company billing & shipping addresses.
•Your phone number, fax number and e-mail address for order
confirmation and invoice.
• The quantity, name and ISBN number of each title.
• Your Tax ID number, if you are tax exempt.
